Mysterious Old Epiphone

I've been all over the Web to identify this one--any ideas? Black solid body, heavy, uneven double cut-out w/ "bat wing" head (like Fender). So far it's a Crestwood or Wilshire from c.1964. BUT--the headpiece says in pearl inlay script "Epiphone by Gibson tm" The inlays are pearl wedges on rosewood. It has three pick-ups,(black, one is wide like a humbucker), only two knobs (V/T) (black metal) and three small toggle switches, each one operates a pick-up. no pick guard. Tremelo bar is missing and it hooked into a spring-loaded box. The edges of the body have white trim.
If the neck wasn't unique I'd guess the body was reworked, maybe so, but I've never seen the wedge inlay or the name script.
