I'm shocked that no one has found one on the Internet yet; seems simple enough to scan it and put it up there. I'm keeping up the lookout, but I'll check eBay, too... I'd hate to pay for the wrong template, though. If you find a template, please post it and let us know in this forum where to find it, and I'll do the same if I find one first.
